In order to receive a player evaluation from your coach you must fill out the Coach Evaluation first, email to while cc'ing your coach.? At the bottom of the sheet you will have to check off if you would like a Player Evaluation.? No Player Evaluation will be given unless a Coach Evaluation is completed first with full disclosure of player submitting sheet.?
